Last month, Amber attended the Discharge Planner’s Breakfast at Melham Medical Center in Broken Bow, an event she makes a priority whenever it’s held. These gatherings bring together hospital discharge planners, care coordinators, and healthcare professionals who play a critical role in helping patients safely transition home after hospitalization or illness.

 

Why Discharge Planning Matters

The goal of discharge planning is simple—but incredibly important: to help individuals live safely and independently at home after leaving the hospital.
